First, there's only one free out-of-system transfer in the entire city (at Lexington Ave-59th St/63rd St). There used to be one at Court Square, but an in-system transfer was built there recently.

When you make that transfer, just walk from one station to the other, then swipe your card at the new station. The system will detect the transfer without you doing anything else (not quite sure how it works, but it does). It will not charge you. That's also how transfers between buses and the subway work (though, since the subway costs more than the bus, you'll be charged the difference when transferring from bus to subway).
